Matrescence

/mæˈtres.ənts/ noun.

— the emotional, mental, physical, social, and physiological transformative process a woman goes through as she becomes a mother.

Matrescence is a term that was first coined in 1973 by Dana Raphael, a medical anthropologist to describe the identity shift that happens as a woman becomes a mother. For some, this happens when trying to conceive. For others, this happens at childbirth. Either way, the process of “matrescence” is a developmental passage where a woman transitions from pre-conception, pregnancy, childbirth, surrogacy, or adoption to the postpartum period, which can last up to 10 years or even a lifetime!

Matrescence is a period of time where a woman lives in the emotional “tug-of-war” — the “push and pull” — the feeling of ambivalence, which is normal and nothing to be ashamed of. It is NOT the same as postpartum depression.

“Matrescence” and “adolescence” sound similar for a reason! They both mark periods of significant transition in a person’s life leading to hormonal and physiological changes and ultimately, a shift in identity.

The year is 1932.

You see a young woman, likely in her early 20s, grimacing in pain trying to breastfeed her newborn with one hand while holding her lower back in the other. She is dressed in her nightgown with noticeable stains (maybe food? maybe breastmilk? maybe poop? who knows?). She had only just given birth 5 days prior. She slowly makes her way to the rotary dial phone in the kitchen to call the doctor.

20 minutes later, the doctor walks in with a heartwarming, compassionate smile and a black leather bag in hand. She evaluates the young woman and provides her with the appropriate treatment. After that, the doctor takes the time to sit next to the young mother, answers all of her questions with patience, and places a hand on her shoulder, reassuring her that she is doing the best she can. After all of her questions are answered, the young mother sees the doctor off with a sigh of relief and the first smile that anyone has seen on her face in days.

Okay, now we’re back to the 21st century — present day.


Remember the good old days when doctors would make house calls when you were injured or feeling sick?

Okay, me neither, because I was definitely not alive in the early 1930s when ~40% of doctor-patient encounters occurred in the patients’ homes, but you get the picture.

Doctors would come directly to your home and could spend as much time with you as necessary in order to care for you and your family. You wouldn’t feel rushed nor have to wait for hours in a waiting room.

According to an American Academy of Family Physicians (AAFP) Survey, only 13% of doctors make regular house calls today. Of those doctors, only 3% make more than 1 house call a week — largely due to the creation of large hospital systems, making house calls inefficient and inconvenient, because as we all know, time = money.

Well, Dr. Iris would like to take you back to the "good old days” — except with modern-day, evidence-based medicine and exceptional care with a holistic mindset.​

Dr. Iris understands that as a new mom or a busy professional living in today’s fast-paced society, it can be difficult to schedule time for appointments to take care of yourself and your body. As a new mom, the last thing you want to do (or are even able to do) is leave your house when you’re sleep-deprived, in your pajamas with your hair in a messy bun, haven’t showered in a week — all with a crying newborn in one hand and a diaper bag filled to the brim in the other.

In today’s society, once a mother enters the postpartum period, an identity shift happens (aka matrescence). She goes from being pregnant, seeing her OB for 15-17 prenatal visits, getting all of the support and attention from friends and family, and even strangers offering her a seat on the subway/train/when standing in line to “Goodbye, good luck, & be well.”

After childbirth, all of the attention immediately turns to the baby — is he/she sleeping, eating, pooping? How many wet diapers has she/he had? Is she/he comfortable?

But what about you & your needs as a new mom?

In our society, it’s as if you go from being a celebrity where everyone asks how you’re feeling to only asking only about your baby. When you’re discharged from the hospital, you go from having all of the support in the world to “Good luck! See you in 4-6 weeks for your postpartum follow-up appointment.” But what society fails to tell you is that those 4-6 weeks are some of the toughest to cope with as a brand new mother.

No wonder why as many as 40% of women do not attend a postpartum visit.

The “pinball effect” of feelings of pure joy to sheer disappointment or failure are thoughts that are common to new moms as they transition into their roles and lose their sense of self & wellbeing.

With the prevalence of social media in today’s society, women feel immense pressure to be “the perfect mom.” It’s as if moms are not allowed to be anything but happy after having a baby.

Unfortunately, despite the fact that 85% of chronic diseases in the United States stem from poor lifestyle choices, most physicians lack the proper foundation in lifestyle-related therapeutic interventions (nutrition, exercise prescriptions, stress management, sleep hygiene) to provide the care that postpartum women need and deserve. On top of that, physicians are poorly trained to diagnose and manage musculoskeletal issues that arise in pregnancy and extend into motherhood, including pelvic dysfunctions leading to urinary incontinence and sacral pain postpartum.
